ATTENTION STUDENTS!
THE DEADLINE for SPRING/SUMMER WAIVERS IS: 1/25/2010
If your waiver form has not been
stamped "approved" on or before the deadline (by Health Center staff), you will
be automatically enrolled
in the
campus Student Health
Insurance Plan.
Spring/Summer 2009 Insurance
Waiver forms are now available at the Health Center. These are for Cal Maritime students who do not wish to be
automatically enrolled in the
campus Student
Health Insurance Plan because they have personal insurance or
are a dependent on their parent or guardian's medical insurance policy which
meets the
minimum insurance requirements to waive.
Important:
If you do not waive the school’s insurance for Spring/Summer 2010,
your
insurance fees
will not be credited .
If you do not need the
campus Student Health Insurance Plan, please stop by the Student Health & Wellness
Center with your insurance card to complete a waiver.
You
must fill out the
2009/10 Medical Insurance Fee Waiver
if
you:
-
are
a transfer or returning student and already have medical insurance;
-
believe any
information on your
last waiver has changed (such as having a new
insurance policy, card, etc.); or
-
did not complete a
waiver last semester and are covered by another policy already (such as
your parent or guardian's) Spring Semester Only .
We cannot accept waiver forms without clear photocopies of the front and back of your
insurance card. Please be sure you have it with you when you stop by.
NOTE:
If you submitted a waiver form last Fall, it is time again to file a new one
- even if all of the information is still current, we need a new form! Students who previously had the
school's insurance are expected to use their old cards; only new policies
will receive a card in Fall.
If you do not have a card,
immediately notify the
SHC via email.
