Student Records:: Veteran's Resources

Students who would like to apply for VA Educational Benefits, or who need to request a change in program or place of training, can obtain VA forms directly from the GI Bill Website.  It is recommended that students apply directly online for initial approval, or if changing schools.   http://www.gibill.va.gov/GI_Bill_Info/education_forms.htm

Otherwise, the student can print out the PDF forms below and mail them directly to the Department of Veterans Affairs.

Form 22-5490 Application for VA Education Benefits (dependent) (pdf)

Form 22-1995 Request for Change of Program or Place of Training (veteran) (pdf)

Once a student becomes eligible (approved) to receive VA Educational Benefits, the student should submit this completed form to the Student Records Office in order to start the Enrollment Certification process.   Cal Maritime Veteran's Information Form (doc)
