Overload Form (pdf) - Required for more than 20 units - Department Chair approval needed.
Return approved form to Student
Records at least one day prior to
registering.

Waiver of Prerequisite Form (pdf) - Required if you haven't completed a particular prerequisite, Department Chair approval needed.
Return approved form to Student
Records after you have registered
for your other courses.
